# Form Templates

> Save reusable question sets for customer information requests.

Form templates help your team ask consistent questions across customer projects. Each task receives an independent copy that you can adapt for that customer.

A form template contains questions and a response policy. A [project template](/product/project-templates) contains the delivery plan, including its tasks and any attached forms.

## Open the library

Go to **Configuration → Project Templates**, then select **Form Templates**. Search by name, description, or creator. The list shows the number of questions, **Draft** or **Published** status, and when a template was updated.

## Create a form template

<Steps>
  <Step title="Choose a starting point">
    Select **Form Template**, then **Create from scratch** or **Create from a template**. Browse saved team templates and starters such as onboarding intake, QBR preparation, or implementation requirements.

    Choosing a starter opens a copy for you to customize.
  </Step>

  <Step title="Build the questions">
    Add a title, description, and questions. Review required answers, choice options, conditions, and the customer response policy. See [Task Forms](/product/task-forms) for the question types and form-building controls.
  </Step>

  <Step title="Save for reuse">
    Choose **Save as draft** to keep unfinished work in the library. Choose **Save template** when it is ready for your team to use. A reusable template needs a name and at least one question.
  </Step>
</Steps>

**Save template** gives the library entry Published status and makes it available for reuse. It does not publish a customer-facing form or start collecting responses.

## Use a template on a task

Open a customer project's task and choose **Form → Add form → Start from templates**. Select a published template, then review and customize the copied questions.

Save and [publish the task form](/product/task-forms#save-and-publish) when it is ready. Configure [customer access](/product/customer-portal) separately.

You can also use a saved form template while adding a form to a task in a [project template](/product/project-templates#include-forms).

## Edit, duplicate, or remove templates

* **Edit:** update the source for future copies. Existing task forms keep their own questions and settings.
* **Duplicate:** open a new editable copy, then save it with its own name.
* **Delete template:** confirm removal from the library. Existing task copies remain available.

<Note>
  Saving an existing Published template with **Save as draft** withdraws it from the template picker. Select **Save template** again when it is ready for reuse. Existing task copies are unaffected.
</Note>

## FAQ

<AccordionGroup>

  </Accordion>

  <Accordion title="Why is saving disabled?">
    Check the title, question labels, choice options, and conditional answers. Save template also requires at least one question.
  </Accordion>

  <Accordion title="Will changing a template update forms already in use?">
    No. Task forms are independent copies. Edit and publish each task form separately when an existing customer's questions need to change.
